Programs Designed to Support Caregivers

Whether you’re taking care of a parent, grandparent, sibling, or friend, we know you want the best for your loved one. The resources below help support you while you support others.

Specially trained teams just for you.

The HOP Administration Unit, customer service teams, program advocates, and case managers are specially trained on Health Options Program benefits. They are here to support you and your family when they’re faced with a serious health issue or a hospital admission, or you want to focus on their well-being.

Elder Care Program

This program connects members with general wellness resources such as meal delivery, local transportation, prescription drug support services, and education on certain health conditions through personalized Care-kits.

For example, if a member is admitted to the hospital, the Elder Care Program connects them with an advocate to see if they will need education and assistance post-discharge for care or follow up services.

Case Management Program

Designed to support members who are going through a serious health condition or treatment program. The Case Management team focuses on:

  • Answering health-related questions and providing education.
  • Coordinating care across providers and services.
  • Guiding members through lifestyle changes related to their medical conditions.
  • Offering ongoing support throughout the course of illness.

Members and caregivers are central to the process, actively participating in individualized care planning and decision-making helps ensure the best possible outcomes. In most cases, a representative will reach out if you have a planned hospital stay or need skilled nursing care.

Medication Therapy Management Program

The Medication Therapy Management (MTM) program is designed to help members better understand their medications, ensure appropriate use, and reduce the risk of adverse events. 

OptumRx, the administrator for the Medicare Prescription Drug Options, will contact eligible members to invite them to participate in the MTM program. If your loved one qualifies for the program and does not opt out, they will be automatically enrolled.
